A Northern Valleys Netball Club registration form must be completed with up to date details and handed in on Registration Day.
Seniors must include their date of birth.
New passport size photographs are required by new players other than nettas, players entering under 10s competition, IJs and senior competition. Please write your name and date of birth on the back of the photo.
Birth Certificates (photocopy) must be sited by committee for all new players 18 years and under. All junior players are to submit their birth certificate number on their registration form

FEES for 2012

Seniors:	$135
Juniors:	$125 -includes umpiring fees
Nettas:	$100 -includes umpiring fees

A family reduction of $10 applies on the second and subsequent registrations.

Fees to be paid at the time of registration unless arrangements have been made with the club treasurer.
